New client forms
New clients scheduling Energy and Soul Illumination Healing Sessions are required to complete the Client Intake and Liability Waiver forms. Please take a few minutes to complete these forms prior to your initial appointment. Forms are available via the drop down box from the navigation menu or by clicking the form button. Read below for some additional instructions on the forms.
Client Intake Form: Pull up the Client Intake form which is an online form. Complete each section, select the submit button, and your information will be submitted online.
Liability Waiver Form: Pull up the Liability Waiver form, read thoroughly, complete the Client Authorization & Signature section, select the submit button, and your information will be submitted online. For clients who prefer retaining a hard copy, simply download the document from the same page and print.
